We the lessees of apartment_____ Rockwood Circle , Middletown, NY, hereby agree to make the following changes regarding the lessees of said apartment, effective_________, 20__.

The following lessees shall be removed from the lease upon the effective date of this addendum:

The following persons shall be added to the lease as lessees upon the effective date of this addendum:

The lessor shall provide a copy of the statement of condition issued to the apartment at the beginning of the original term so that the old and the new lessees are clear as to the original condition of the apartment. The lessor will not be responsible for any damage caused by the lessees and will hold the new lessees responsible for damage cited at the termination of the occupancy. We strongly recomend that a determination of existing damage be reached and compensation made to the incoming lessees. Normal wear and tear is expected.

The security deposit and last month’s rent will be transferred to the new lessees upon the effective date of this addendum. The new household acknowledges that they have reimbursed the outgoing lessees for their share of the security deposit and last month’s rent shall be issued upon the effective date of this addendum.
